Our role is to help you reconnect with what matters most. By identifying the everyday activities that may have become difficult, we provide practical strategies and tailored solutions to make daily life easier, safer, and more meaningful.

Our approach goes beyond short-term fixes. We focus on creating lasting change, giving you the tools and confidence to keep thriving long after our sessions. Whether it’s showering independently again, returning to your beloved garden, or simply enjoying the routines you once took for granted; we’re here to support older Australians in living life to its fullest, in a way that reflects your true essence.

About Us

As the principal occupational therapist, Flynn Chiem has experience working with older adults across the lifespan to promote independence in their home and in the wider community. He holds a Bachelor of Occupational Therapy (With Honours) from The University of Queensland.

In his service, Flynn has always strived to build a strong patient-therapist relationship and prides himself on being a strong communicator. Flynn's honest approach to care allows him to develop a healthy relationship with clients, their families and other service providers. By focusing on the individual, Flynn aims for all recommendations to provide lasting impacts to the person, the spaces that they interact with and the tasks that they need to perform.

When Flynn is not working, he enjoys spending time with his dogs and family, staying fit and healthy (through exercise and good food - he's always looking for new recipes!) and watching the Wests Tigers win.

We are a mobile occupational therapy provider, so we can travel to your home and complete assessments on site.

    Funding

    We are currently accepting referrals for older adults across Home Care Package (HCP) and National Disability Insurance Scheme (NDIS) funding models

    Your Home Care Package case manager can refer to us directly to provide assistance with your home care needs.

    NDIS

    NDIS

    You can make a referral to us directly using your NDIS funding to pay for Occupational Therapy services with no gap. We are only accepting self-managed and plan-managed NDIS participants at this time.

    Services Provided

    Some of the services we provide include:

    Functional Home Assessments

    Identifying barriers and recommending modifications for safety and accessibility.

    Equipment prescription

    Assess and recommend for equipment such as powered mobility devices, walkers, wheelchairs, hospital beds, pressure mattresses, shower and toileting equipment etc.

    Major and minor home modifications

    Assist with access improvements to promote clients to remain at home safely. Common areas are toilets, bathrooms and other problem access points to the home

    Mobility and falls assessments

    Assessing risks and implementing interventions to reduce falls. We can provide education and also recommend suitable aids to minimise any identified concerns

    Cognitive assessments

    We can complete cognitive screens to identify if any deficits may exist. Where required, assistance is developing routines and strategies for memory and concentration can be provided.

    Advocacy

    Provision of detailed reports and letters of recommendations to support alternative housing recommendations, equipment requests or functional capacity evaluations for NDIS access requests and plan reviews
